People who live with hidden disabilities may choose to wear the sunflower symbol via a lanyard to voluntarily show they may need more assistance using essential services in daily life.

You'd have to ask the SEQTA folks to get a full list, but the first that comes to my mind is around disabled preferential seating, and ensuring that other passengers move out of the way to allow people with hidden disabilities to sit down.
